Part 1 · Use of the website
1. Acceptance of these terms
The ELNA Private Care website (the “website”) is operated by ELNA Medical Group Inc. (“ELNA”, “we”). By using the website, you accept these terms and conditions and the privacy policy, which forms part of them. If you do not accept them, please do not use the website.
We may change these terms. The version in force is the one published on this page, with its update date. A change does not apply to a package or membership already purchased unless you agree to it.
2. General information, not medical advice
Website content is provided as general information. It does not replace a consultation, an assessment or advice from a healthcare professional. Consult a physician or another qualified professional before making a decision about your health.
In an emergency, call 911 or go to the nearest emergency department. For non-urgent health advice, you can reach Info-Santé at 811, 24 hours a day.
3. Accuracy of information and prices
We make reasonable efforts to keep website information accurate and up to date, including services, hours and prices. Prices are in Canadian dollars. The price that applies to your visit or package is the one our team confirms to you when you book or purchase.
Prices for services offered by partners, such as the collective-prescription services of CDL Laboratories, are set by those partners and may change.
4. Partners and third-party websites
The website links to third-party websites, including testlabo.ca and cdllabs.com (CDL Laboratories) and Google Maps. Those websites are governed by their own terms and privacy policies. Services booked with CDL Laboratories are provided by CDL Laboratories, in partnership with ELNA.
A link to a third-party website does not mean we endorse all of its content. We are not responsible for the content or operation of those websites.
5. Permitted use
You may use the website for personal and lawful purposes. It is prohibited, in particular, to:
- access or attempt to access content, data or systems not intended for you;
- interfere with the operation or security of the website, including by a virus, a cyberattack or deliberate overloading;
- use a robot or other automated means to copy website content without our written permission;
- impersonate ELNA, a member of our staff or another person;
- transmit false or misleading information, or illegal, hateful or harassing content.
6. Intellectual property
Website content (text, graphics, logos, layout and design) belongs to ELNA or its licensors and is protected by intellectual property laws. The ELNA names and logos are trademarks of ELNA or its affiliates. The CDL Laboratories names and logos belong to their owner.
You may view the website and print a page for personal use. Any other reproduction, modification or distribution, in whole or in part, requires our written permission.
7. Personal information and communications
We protect your personal information in accordance with Québec’s Act respecting the protection of personal information in the private sector and the rules that apply to medical records. The privacy policy explains what information we collect, why, and how to exercise your rights of access and correction.
The website has no medical information forms. Measurement and advertising cookies are only turned on with your consent, and never on our pages about a specific health condition; the privacy policy describes them. Ordinary email is not secure: do not send us sensitive medical information by email. An email or information request is not an appointment.
8. Website availability
We may change, suspend or remove all or part of the website, including for maintenance or security reasons. We do not guarantee that the website will always be available, uninterrupted or error-free.
9. Liability
To the extent permitted by law, ELNA is not liable for indirect damages arising from use of the website, inability to use it, or use of a third-party website.
Nothing in these terms limits ELNA’s liability for bodily or moral injury, for intentional or gross fault, or the rights granted to you by Québec’s Consumer Protection Act or the Civil Code of Québec.
Part 2 · Services, membership and packages
10. Nature of the services
ELNA Private Care offers private primary care from age 14, subject to availability and each service’s criteria. The laboratory welcomes patients from age 3.
ELNA Private Care consultations are paid services outside RAMQ. The public specialist clinic operates separately: its care is covered by RAMQ for eligible patients, requires a medical referral and depends on availability. ELNA membership does not guarantee GMF registration, priority access or a specific wait time to see a specialist.
Care and treatment depend on your clinical assessment. No particular diagnosis, treatment, prescription or result is guaranteed.
11. Appointments
Appointments are booked by phone at 514-787-0055. An appointment is only confirmed once our team has confirmed it with you. Collective-prescription services are booked online with CDL Laboratories at testlabo.ca.
For a minor urgent concern, an appointment can generally be offered within 24 hours, or the same day for a request received before noon. This timeframe is a target, not a guarantee. This service does not replace the emergency department.
12. Annual membership and à la carte prices
The individual $129 annual membership is optional. It gives you the member price on à la carte medical services (health assessments, medical consultations and minor urgent-care visits): $60 less per visit. It does not apply to CDL laboratory testing. It is valid for 12 months from the date of purchase. If you choose a package, the price includes the annual membership.
À la carte visit prices: minor urgent concern or follow-up (20 min): $175 member, $235 non-member; complex follow-up (30 min): $225 member, $285 non-member; health assessment and care plan (about 45 min): $285 member, $345 non-member.
Without a membership, each à la carte visit costs $60 more. The membership pays for itself from the 3rd à la carte visit in a year.
The membership is personal and cannot be transferred.
13. Packages
The content and price of each package (Alliance Santé, Alliance Signature, Équilibre 360, ADHD and ADHD+) are shown on the service page and the pricing page. A package price includes the annual membership and administration fees.
Each package is intended for one individual and is valid for one year. Your doctor recommends bloodwork based on your specific needs. Each package includes one set of laboratory analyses. Additional testing is billed separately.
Alliance Signature. Unlimited visits cover medically necessary appointments for the member themselves, subject to availability. The clinician determines how often follow-ups are needed.
Family discount. If you have a valid package, your spouse or an immediate family member aged 14 or older receives 10% off the purchase of their own package.
Transferable follow-up visit. You can give one of your follow-up visits to a person of your choice aged 14 or older who lives in Québec. The visit is for a consultation about a single health concern; it cannot be used for a comprehensive health assessment.
Included test (Alliance Santé and Alliance Signature). One collective-prescription test per year at any CDL location, up to a value of $150. If the service you choose costs more, you pay the difference.
14. Collective prescriptions and CDL Laboratories
Testing for urinary tract infections, pregnancy, strep throat, sexually transmitted infections (STIs) and HIV is available without a physician’s requisition under collective prescriptions. Book an appointment with a nurse, who assesses eligibility and performs or arranges the appropriate testing. Treatment is prescribed when required, according to the applicable protocol.
These services are offered in partnership with CDL Laboratories, at CDL locations. Each service starts with an eligibility check. If your situation does not meet the collective-prescription criteria, you are directed to a consultation. CDL Laboratories’ booking terms apply.
15. Payment, receipts and insurance
ELNA Private Care services are paid by the patient. We provide receipts for eligible services and offer direct claims with participating insurers; you then pay the portion not covered.
Coverage depends on your insurance plan and is not guaranteed. Check with your insurer before your visit. Some expenses may qualify for the medical expense tax credit; keep your receipts.
16. Cancellation, renewal and refunds
Terms for cancelling or changing an appointment, for renewal and for refunds of a membership or package, as well as accepted payment methods, are given to you before you book or purchase. To change or cancel an appointment, call our team as early as possible.
These terms apply subject to the rights granted to you by Québec’s Consumer Protection Act.
17. Employer offers
An offer presented to an employer is governed by a separate agreement with that employer. The employer and its human resources have no access to your results, diagnoses or reasons for consultation.
General provisions
18. Governing law
These terms are governed by the laws of Québec and the federal laws of Canada that apply there. Subject to the rights granted to you by law, any dispute falls under the jurisdiction of the courts of Québec.
If a provision is found invalid, the others remain in force. Not exercising a right is not a waiver of that right. If the French and English versions differ, the French version prevails.
