Annual membership · $129

The individual $129 annual membership is optional. It gives you the member price on à la carte medical services (health assessments, medical consultations and minor urgent-care visits): $60 less per visit. It does not apply to CDL laboratory testing. It is valid for 12 months from the date of purchase. If you choose a package, the price includes the annual membership.

Without a membership, each à la carte visit costs $60 more. The membership pays for itself from the 3rd à la carte visit in a year. Additional tests or services cost extra.
